Everything is online, so you can finish your forms from home. In early 2027, we’ll send you a friendly email with clear details and easy instructions for the forms your camper needs. These may include:
- Camper profile form
- Camper medical and health history form
- Immunization record
- Medication authorization (if needed)
- Allergy action plan (if needed)
- Allergy and dietary information
- Transportation form
- Authorized pickup form
- Trip permission slips (if needed)
